Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

08B-38-4    Basic - Observed Food stored on floor. Storage shed behind establishment had bags of onions, rice and cartons of oil on the floor. **Repeat Violation**

25-05-4    Basic - Observed Single-service articles (Toilet paper and to go containers) improperly stored on floor in storage shed behind establishment

08B-54-4    Basic - Observed uncovered food ( Tortilla bowls) stored near ice machine on dry goods racka exposed to dust or contamination
02D-01-5    Basic - Observed working containers of food ( Rice and Beans)removed from original container not identified by common name.
12A-16-4    High Priority - Observed Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Employee returned from a smoke break and wet left hand and put on gloves. Employee washed left hand for less than 5 seconds, did not use soap, or allow water to get warm. **Warning**
12A-07-5    High Priority - Observed Employee failed to wash hands before putting on gloves to initiate a task working with food. Employee touched an insecticide can to place in the correct location and changed gloves without washing hands. **Warning**
12A-11-4    High Priority - Observed Employee used the bathroom and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands upon returning to employee area. Inspector advised employee to wash hands correctly and put on gloves. **Warning**
41-05-4    High Priority - Observed Pesticide/insecticide labeled for household use only present in establishment.
35A-02-6    High Priority - Observed two Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area.
16-37-1    Intermediate - Observed Chlorine No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Three compartment sink sanitizer solution measured 0 ppm of chlorine.
22-02-4    Intermediate - Observed Food-contact surface ( Cutting board in prep area) soiled with food debris, mold-like substance or slime.
31A-11-4    Intermediate - Observed Handwash sink used for purposes other than handwashing. Scoops found in hand wash sink in kitchen next to prep table.
53A-05-6    Intermediate - Observed No currently certified food service manager on duty with four or more employees engaged in food preparation/handling.
