Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-22-4    Basic - Floor area(s) covered with standing water. In back kitchen area.
36-73-4    Basic - Floors, walls, and/or ceilings soiled/has accumulation of debris. Floor around front counter area. Floor under refrigeration equipment in front area. Floor in hallway to restroom. Floor under equipment in kitchen area. Floor in walk in cooler (live crabs). Wall behind cooking equipment. Wall around triple sink area. Ceiling in kitchen area. **Repeat Violation**

33-29-4    Basic - Grease receptacle lid open, broken, or missing. Outside grease receptacle in back of establishment.

10-01-5    Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Scoops for spices at front counter.
02D-03-4    Basic - Label on a food item prepared and packaged onsite for customer self-service does not include product identity/description, date product was packaged and name and address of the establishment that prepared and packaged the food. Cupcakes in grab and go cooler in front area.
05-09-4    Basic - No conspicuously located ambient air temperature thermometer in holding unit. In oyster reach in cooler in kitchen area. **Repeat Violation**
14-47-4    Basic - Nonfood-contact equipment exposed to splash/spillage not constructed of materials that are corrosion resistant, non-absorbent, smooth and easily cleanable. Front of counters below microwaves are cracked. Rust present at bottom and interior of walk in freezer. Shelves below prep tables in kitchen. **Repeat Violation**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Exterior of spice bins at front counter. Floor fan near front counter. Multiple handles for reach in coolers in kitchen. Gaskets for multiple reach in coolers in kitchen. Gasket for walk in freezer. Exterior of cooking equipment in kitchen. 35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Three live flies observed in kitchen area.
29-37-4    High Priority - Spray hose at dish sink lower than flood rim of sink. Black hose in triple sink in kitchen.

51-16-7    Intermediate - No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days.
