Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

08B-49-4    Basic - Employee personal food not properly identified and segregated from food to be served to the public. Observed employee drinks stored inside salad cooler in kitchen area not properly segregated from establishment.
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed employee bottle of water stored on top clean prep table located by front counter. **Corrected On-Site**
13-03-4    Basic - Employee with no hair restraint while engaging in food preparation. Observed employee engaging in food preparation with no hair restraint.
14-11-4    Basic - Equipment in poor repair. Observed standing cooler gasket disrepair located in kitchen area.
08B-38-4    Basic - Food stored on floor. Observed case of oil stored on floor by the back door.
14-69-4    Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Located inside small white freezer located by small cafeteria area.
22-08-4    Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed interior of oven soiled.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed all cooler gasket soiled.
14-33-4    Basic - Reach-in cooler shelves with rust that has pitted the surface.
08B-12-5    Basic - Stored food not covered. Observed ham croquettes stored inside walk in freezer not cover.
14-17-4    Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface.
50-17-2    High Priority - Operating with an expired Division of Hotels and Restaurants license. **Admin Complaint**
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw beef stored over cooked chicken inside standing cooler in kitchen area. Operator moved raw beef to bottom shelves. **Corrected On-Site** **Repeat Violation**
41-10-4    High Priority - Toxic substance/chemical improperly stored. Observed bottle of glass cleaner stored on top prep table located at front counter. Operator moved to designated area. **Corrected On-Site**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed slicer with old food residue located in kitchen area.
02C-02-5    Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed cut ham and slice cheese stored inside walk in cooler not date.
