Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

24-06-4    Basic - Clean utensils or equipment stored in dirty drawer or rack. Observed pots and pans stored on soiled bottom shelf.
35A-03-4    Basic - Dead roaches on premises. Observed 2 dead roaches in kitchen area, approximately 6 dead roaches underneath the kitchen equipment.
16-13-5    Basic - Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized.
14-11-4    Basic - Equipment in poor repair. Refrigerator units in poor repair.
36-01-4    Basic - Floor not cleaned when the least amount of food is exposed.
36-73-4    Basic - Floor soiled/has accumulation of debris. Under the kitchen equipments.
06-06-5    Basic - Frozen time/temperature control for safety food being slacked at room temperature is no longer frozen solid. Observed fish 52°F, in 3 compartment sink.
36-24-5    Basic - Hole in or other damage to wall. In kitchen area.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed kitchen preparation table soiled.
22-16-4    Basic - Reach-in cooler interior/shelves have accumulation of soil residues.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust.
41-05-4    High Priority - Pesticide/insecticide labeled for household use only present in establishment.
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Eggs over cooked rice inside reach in cooler.
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed raw fish 52°F, slaking inside 3 compartment sink.
41-25-4    High Priority - Tracking powder pesticide used inside establishment.
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed cutting boards, food containers inside reach in coolers soiled.
02B-01-5    Intermediate - Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Menu offers fish ceviche, no identified on menu as a raw or undercooked animal product. Establishment have consumer advisory posted at front counter.
14-04-4    Intermediate - Microwave missing fan guard cover/splatter shield.
02C-02-5    Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed cooked tice from previous days in reach in cooler.
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled.
