Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-36-4    Basic - Ceiling tile missing. In back hallway of kitchen storage. Some tiles are also in disrepair throughout kitchen.
35A-03-4    Basic - Dead roaches on premises. Observed four dead roaches in establishment: three in small prep kitchen and one under front counter. Dead roaches discarded. **Corrected On-Site**
35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. Side exit door in small kitchen.
36-73-4    Basic - Floors, walls, and/or ceilings soiled/has accumulation of debris. Floor up under mop sink. Floor under reach in coolers in back of kitchen. Floor behind hot water heater in back storage area. Ceiling tiles in kitchen area by vents. **Repeat Violation**
14-42-4    Basic - Hood filter missing from automatic fire suppression/exhaust system. Gap in hood filters above cooking equipment indicates need for another filter or spacer.
38-07-4    Basic - Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. Light in main kitchen area.
14-47-4    Basic - Nonfood-contact equipment exposed to splash/spillage not constructed of materials that are corrosion resistant, non-absorbent, smooth and easily cleanable. Unsealed wood at front counter.
33-16-4    Basic - Open dumpster lid. Outdoor dumpster.
29-08-4    Basic - Plumbing system in disrepair. Faucet for mop sink is not connected. **Repeat Violation**
25-05-4    Basic - Single-service articles improperly stored. To go containers on floor in storage area in small kitchen. Containers moved and stored correctly. **Corrected On-Site**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 11 live flies in establishment: eight in small prep kitchen area near exit door and three in back storage area near hot water heater. **Warning**

35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Observed 32 rodent droppings in establishment: 17 in back storage area behind hot water heater. Area contains reach in cooler and reach in freezer and is adjacent to main kitchen area, and 15 in old walk in freezer converted to storage area with food and to go items adjacent to kitchen. 03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. In reach in cooler in back hallway farthest from kitchen, cooked chicken, raw bacon, cooked potatoes 59°F. Employee states items placed in cooler approximately 2.5 hours prior. Items moved to another reach in cooler to cool. **Warning**
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Missing on outside faucet on front of building where black hose is attached. Missing on faucet in corner of small kitchen near storage area door. **Repeat Violation** **Admin Complaint**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Interior of fryer in main kitchen.
53A-02-7    Intermediate - Manager or person in charge lacking proof of food manager certification and no other certified food service manager employed at this location. Only certified food manager expired 03/16/2021. **Warning**
16-37-1    Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. No kit available.
