Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

29-18-4    Basic - Drain cover(s) missing. Observed drain cover missing underneath the three compartment sink and on the floor nearby the hand washing sink.
13-03-4    Basic - Employee with no hair restraint while engaging in food preparation. Observed the employee in the kitchen without a hair restraint. **Corrected On-Site**
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed the floor soiled in the kitchen underneath the cooking equipment and reach in coolers. **Repeat Violation**
08B-38-4    Basic - Food stored on floor. Kitchen: observed a container with sugar on the floor in front of the three compartment sink. **Corrected On-Site** **Repeat Violation**

23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed in the kitchen: hoods and filters soiled with grease and soap and paper towel dispenser soiled. Observed the front counter hand wash sink soiled with black substance.
22-16-4    Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Observed all coolers and chest reach in freezer in the kitchen soiled.
33-34-4    Basic - Storage area not maintained clean and organized. Dry storage area and front counter.
33-31-5    Basic - Unnecessary items/unused equipment on the premises. Observed in the kitchen: 2 non-working reach in coolers.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed throughout the kitchen: walls with sauce and food debris. **Repeat Violation**
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Observed in the kitchen: wet wiping cloth stored on the prep table. **Corrected On-Site**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Kitchen: container of sugar not labeled.
50-17-2    High Priority - Operating with an expired Division of Hotels and Restaurants license. Observed license expired 10/01/21.
08A-04-5    High Priority - Raw animal food stored over or with unwashed produce. Front counter, stand up glass reach in cooler: raw shell eggs stored above bell peppers. **Corrected On-Site**
53A-01-7    Intermediate - Manager or person in charge lacking proof of food manager certification.
16-37-1    Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed operator could not provide chlorine test kit during the inspection.
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Kitchen hand washing sink. **Corrected On-Site**
05-08-4    Intermediate - No probe thermometer provided to measure temperature of food products.
53B-01-5    Intermediate - No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233.
22-38-5    Intermediate - No sanitizer of any kind available for warewashing. Only use single-service items to serve food to customers until sanitizer is available for warewashing. Observed employee go to a nearby store to by chlorine. **Corrected On-Site**
31B-03-4    Intermediate - No soap provided at handwash sink. Kitchen.
02B-02-5    Intermediate - Raw/undercooked animal food offered and establishment has no written consumer advisory. Raw animal foods must be fully cooked prior to service. Observed the establishment offers Bridgewater Conch Salad (fresh conch). Discussed with the operator and emailed the Consumer Advisory handout to the operator. **Corrective Action Taken** **Repeat Violation**
