Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

24-05-4    Basic - Clean glasses, cups, bowls, plates, pots and pans not stored inverted or in a protected manner. Observed plates at front line on various shelving units and at server terminal near bar area. **Warning**
35A-03-4    Basic - Dead roaches on premises. Observed 2 dead roaches an one egg case at Pepsi soda machine at front counter. Operator stated this soda machine is not in use. Advised operator to discard dead roaches and sanitize area. **Warning**
13-03-4    Basic - Employee with no hair restraint while engaging in food preparation. Observed several cooks engaging in food preparation without hair nets. Advised and educated operator. **Warning**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed soiled hood/ hood filters. **Warning**

02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed stacked containers of flour and panko not labeled on shelving unit next to flip top at cook line. **Warning**
12A-10-4    High Priority - Employee touched bare body part and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Observed cook on phone then proceeded to continue cutting raw beef. Instructed employee to wash hands before resuming food preparation. **Warning**
50-17-2    High Priority - Operating with an expired Division of Hotels and Restaurants license. Observed expired today 12/01/2021. **Warning**
02C-01-5    High Priority - Ready-to-eat, time/temperature control for safety food marked with a date that exceeds 7 days after opening/preparation. Observed opened half and half with expiration date of 11/28/2021 at bar reach in cooler. See Stop Sale. **Warning**

Observed hot water handle not functioning at bar hand sink next to right of sushi show case. Observed hand sink to right of triple sink at bar area not operable.
