Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-03-4    Basic - Dead roaches on premises. Observed 18 dead roaches in establishment: two under front counter, 15 under triple sink in kitchen and one in women's restroom. All dead roaches discarded. **Corrected On-Site**

14-47-4    Basic - Nonfood-contact equipment exposed to splash/spillage not constructed of materials that are corrosion resistant, non-absorbent, smooth and easily cleanable. Shelves in chemical storage area are rusted.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Trash can at front counter. Exterior of oven and fryer on cook line. Multiple reach in cooler and reach in freezer gaskets in kitchen area. Speed rack in prep area. **Repeat Violation**
33-16-4    Basic - Open dumpster lid. Outside dumpster near rear door.
12B-13-4    Basic - Opened employee beverage container in a cold holding unit with food to be served to customers. Two employee drinks in reach in freezer in prep area. Drinks removed. **Corrected On-Site** **Repeat Violation**
35B-03-4    Basic - Outer openings not protected with self-closing doors. Rear exit door does not self close.
36-02-5    Basic - Unsealed concrete floor in food preparation, food storage, warewashing area or bathroom. Throughout kitchen area.
36-30-4    Basic - Wall not smooth, nonabsorbent and easily cleanable in food preparation, food storage, warewashing areas or bathroom. Multiple wall patches made with wood not sealed.
08A-02-6    High Priority - Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. Raw bag of chicken stored over raw pork in reach in freezer in prep area. Items moved and stored correctly. **Corrected On-Site**

53B-01-5    Intermediate - No proof of required state approved employee training provided for any employees.
