Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Microwave soiled. Observed hood soiled. Observed fryer with heavy accumulation of grease. Observed reach in cooler handles soiled.
16-46-4    Basic - Old labels stuck to food containers after cleaning.
22-16-4    Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Kitchen area.
29-49-6    Basic - Standing water in bottom of reach-in-cooler located across from 3 compartment sink.
06-01-5    Basic - Time/temperature control for safety food thawed in an improper manner. Observed calamari thawed in standing water at the prep area.
36-04-5    Basic - Utility lines are unnecessarily exposed.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Kitchen area walls behind stove and next by hand sink.
29-11-4    Basic - Water leaking from pipe and/or faucet/handle. Hand sink pipe leaking located in the restroom. Observed hand sink pipe leaking at the front area.
50-17-2    High Priority - Operating with an expired Division of Hotels and Restaurants license. **Admin Complaint**
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw beef stored over cooked potatoes.
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed raw chicken (50 - Cold Holding) stored in the reach in cooler located at the kitchen area.
41-10-4    High Priority - Toxic substance/chemical improperly stored. Observed Tylenol stored on shelf by seasonings.
02B-01-5    Intermediate - Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Observed on the online menu, establishment serves ceviche as per operator ceviche is made with raw fish, this plate is not under consumer advisory.
16-37-1    Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Chlorine test strips.
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink located in the kitchen area. **Corrected On-Site**
02C-02-5    Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed cooked rice, cooked potatoes with no date marked.
