Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. Observed gap beneath exterior back door. Outer openings not protected with self-closing doors. Observed exterior back door with no self closing device attached to door. **Warning**
36-17-5    Basic - Floor tiles missing and/or in disrepair. Observed broken missing tiles in dish ware area. **Warning**
35B-08-4    Basic - Fly sticky tape hanging over food/food preparation area/food-contact equipment. Observed 7 sticky fly adhesive device installed over prep areas , back door, with approximately 15-20 dead flies stuck to adhesive devices. **Warning**
10-01-5    Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed handles of scoop directly touching msg, sugar, flour, salt, rice in containers located in kitchen areas In-use utensil not stored with handle above the top of time/temperature control for safety food and rim of the container. Observed in use utensils handle touching shrimp. Cooked macaroni and cheese, cooked ribs in walk in cooler. Operator removed and stored correctly. In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed utensils stored in standing water at hibachi station in water at 110°F. Operator removed and stored in hot water 145°F. In-use tongs stored on equipment door handle between uses. Observed in use utensils stored on equipment door handles. 36-64-5    Basic - Objectionable odors in bathroom or other areas of the establishment. Observed objectionable odors coming from restroom, kitchen area and walk in cooler. **Warning**

29-49-6    Basic - Standing water in bottom of reach-in-cooler. Observed standing water in 3 reach in coolers Located at hibachi station and kitchen area. **Warning**
08B-12-5    Basic - Stored food not covered. Observed several containers of cooked chicken, cooked flan, crab, breaded shrimp soy sauce, egg rolls , not covered in walk in cooler. Food stored on floor. Observed oil, cut celery in bus pan being stored on floor in kitchen area by cook line. **Warning**
06-01-5    Basic - Time/temperature control for safety food thawed in an improper manner. Observed shrimp stored in standing water in 3 compartment sink. **Warning**
21-09-4    Basic - Wet wiping cloths used for occasional spills on equipment food- and nonfood-contact surfaces not clean. Observed soiled wet cloths on cooks counter being used to clean food contact surfaces without sanitizer solution. Wet wiping cloth not stored in sanitizing solution between uses. Observed wet wiping cloth stored on counter tops not stored in sanitizing solution. Sanitizer bucket stored with food, equipment, utensils, linens, single-service, or single-use articles. Observed sanitizing solution bucket stored on floor in kitchen area. **Warning**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed sushi rice, jasmine rice, brown rice, self rising flour salt,sugar, not labeled **Warning**
01B-01-4    High Priority - Dented/rusted cans present. See stop sale. Observed 2 dented can bamboo shoots 12lbs 16 0z **Warning**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 2 live flies flying around kitchen next to 3 compartment sink and cook line not landing on any product. Operator called pest control and killed two flies. **Corrected On-Site** **Warning**

Observed raw beef stored over uncovered celery in walk in cooler. Raw animal food stored over or with unwashed produce. 22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed cutting board, ovens, can blade and ice bin interior with mold like substance. 31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Observed hand washing sink located by3 compartment sink with spoon inside sink. Operator removed and stored correctly. **Warning**
02C-02-5    Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed Neptune salad and cooked pork per operator made on April 10th no date marking **Warning**
