Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-73-4    Basic - Floor soiled/has accumulation of debris. Throughout the bar, under kitchen equipment and under the reach in coolers on cook line. **Warning**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed 13 live roaches coming from underneath one of the reach in coolers on the cook line. One live inside the reach in cooler. The cooler is under the flat top. One live between gaskets in reach in cooler location in front of flat top. Two under reach in cooler located in front of the ovens on the cook line. Two live crawling near server station. **Warning**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Walk in cooler D: raw chicken 46°F. Observed cooler holding temperature at 45°F. Per chef walk in cooler was checked in the morning, record was 38°F, chef believes cooler is under defrost. Advised to keep door closed or move the chicken to different walk in cooler. **Corrective Action Taken** **Warning**
31A-10-4    Intermediate - Equipment drain line draining into handwash sink. Hand sink in banquet kitchen. Observed hand sink on server station being used as a dumping sink. **Warning**
