Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-05-4    High Priority - - From initial inspection : High Priority - Roach activity present as evidenced by live roaches found. 5 live roaches on bag in box soda rack. Observed 10 live roaches behind ice machine in kitchen in and on wall. Employee removed a cardboard box top from under ice machine and approximately 20 roaches scattered. - From follow-up inspection 2021-08-25: 1 live roach on desk at entrance into establishment. Operator exterminated live roach then cleaned and sanitized area. 1 dead under handwash sink area next to three compartment sink. 1 dead roach at base of water heater, egg sac at base of water heater as well. 8 dead roaches under shelf of prep table next to walk-in cooler. 1 dead roach under handwash sink of front line. 4 dead roaches under cashier counter in wooden cracks of front line. 1 dead roach in drain under ice machine. Operator cleaned up some of the dead roaches. **Time Extended** **Corrective Action Taken**
