Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

16-55-4    Basic - Dishmachine not washing/rinsing properly. Must wash, rinse and sanitize all dishware, equipment and utensils in three-compartment sink until dishmachine is functioning properly. Dish machine sanitizer reading 0ppm. Discussed with manager about having a repairman out and use of 3 compartment sink for rest of day.
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Employee belongings on prep surface across from batter station. Cook moved to appropriate storage location. **Corrected On-Site**
35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. Door in dish area is unable to be properly secured to prevent entry of pests due to gap at base.
36-22-4    Basic - Floor area(s) covered with standing water. Standing water on floor at entrance to ice machine room in rear of establishment.
36-17-5    Basic - Floor tiles missing and/or in disrepair and/or in disrepair. Floor tiles missing near stand up Reach in cooler between dish area and cook line.
08B-38-4    Basic - Food stored on floor. Buckets with food items inside stored on floor in walk in cooler. Manager had cooks move to proper storage location. Food stored on floor near in walk in freezer. **Corrective Action Taken** **Repeat Violation**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Hoods over flat top on cook line have an accumulation of grease buildup. Fans in walk in cooler have an accumulation of dust on fan shields. **Repeat Violation**
36-64-5    Basic - Objectionable odors in bathroom or other areas of the establishment. Strong odor present in bar area near beer cooler along back wall.
42-03-5    Basic - Storage of maintenance equipment/tools in areas that may result in cross contamination of food, equipment, utensils, linens and/or single-service and single-use articles. 21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Multiple wet wiping cloths found on surfaces throughout kitchen areas and bar. Manager properly stored in sanitizer. **Corrected On-Site**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. 2 large bins with not easily identifiable items inside, beneath batter station not labeled with common name of item. Cook properly labeled items. **Corrected On-Site**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 3 live small flying insects in kitchen area. In soda storage room located In dining room area near service station found 6 rodent droppings on floor under soda racks, 9 rodent droppings on soda boxes, and 5 rodent droppings on shelf. In back ice machine room found 4 rodent droppings on floor beneath larger ice machine.
41-10-4    High Priority - Toxic substance/chemical improperly stored. Spray bottle of chemicals stored on prep surface across from fryers. Box of grill cleaner stored on shelf with food items in corner of cook line by fryers. Cook moved all items to proper storage locations. **Corrected On-Site**
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. No vacuum breaker installed on for hose in dish area on hot water line behind three compartment sink. **Repeat Violation** **Admin Complaint**
01C-03-4    Intermediate - Clam/mussel/oyster tags not marked with last date served. Tags marked with date prior to last oyster being sold.
02C-03-5    Intermediate - Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Milk in stand up Reach in cooler between dish and cook line has opened containers of milk not date marked. Employee stated that the milk had been opened for 2 days prior. Employee properly date marked the milks. Container of milk in walk in cooler not date marked. Manager stated that it had been open for over 24 hours and had an employee properly date mark. **Corrected On-Site**
01C-04-4    Intermediate - Container of raw pre-shucked clams/mussels/oysters does not have a proper label. Shucked oysters not labeled. Cook returned label to oysters. **Corrected On-Site**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Both ice machines in rear of establishment have an accumulation of mold like substance on interior of unit.
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Hand wash sink on expo line has sanitizer bucket in well. Manager moved to appropriate location. **Corrected On-Site**
