Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

12B-02-4    Basic - Employee eating in a food preparation or other restricted area.
36-01-4    Basic - Floor not cleaned when the least amount of food is exposed. Observed grease and food debris encrusted on the floor under stove and at fryer station.
08B-38-4    Basic - Food stored on floor. Observed plastic bottles of cooking oil stored on kitchen floor.
14-69-4    Basic - Ice buildup in reach-in freezer and/or walk-in freezer. **Repeat Violation**
10-08-5    Basic - Ice scoop handle in contact with ice in a container in reach in freezer at front counter.
10-07-4    Basic - In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed ladle in water at 75°F. Operator discarded water.
22-08-4    Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris.
14-34-4    Basic - Interior of reach in freezer with rust that has pitted the surface
14-36-5    Basic - Interior of refrigerator or freezer in disrepair/has exposed insulation. Freezer in the kitchen area is taped up with duct tape. **Repeat Violation**
51-13-4    Basic - No Heimlich maneuver/choking sign posted. Provided a copy to the operator. **Corrected On-Site**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed hood filters soiled with grease.
14-33-4    Basic - Reach-in cooler shelves with rust that has pitted the surface. Three door cooler at cook line. **Repeat Violation**
21-44-1    Basic - Sanitizer bucket stored with food, equipment, utensils, linens, single-service, or single-use articles. Observed sanitizer bucket with chlorine stored on shelf with cereal at front counter and on table with slicer in the kitchen.
29-20-5    Basic - Standing water or very slow draining water in handwash sink at front counter. **Repeat Violation**
06-01-5    Basic - Time/temperature control for safety food thawed in an improper manner. Observed fish fillets thawing at room temperature in the kitchen. Operator stored in reach in cooler.
29-03-4    Basic - Water draining onto floor surface. Observed plumbing in disrepair and water draining on the floor at hand wash sink next to three compartment sink.
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw packaged ground beef stored over open package of shredded cheese, and box of raw chicken over open box of cooked sausage in reach in freezer. **Repeat Violation**
02C-01-5    High Priority - Ready-to-eat, time/temperature control for safety food marked with a date that exceeds 7 days after opening/preparation. Observed chicken soup, gravy, cooked on previous day without date marking.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed 10 under stove at cook line, under reach in freezers and coolers in the kitchen, under prep table at cook line.
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Beverage Air three door cooler- sausage, beef franks, gravy, ham, cooked spinach, turkey sausage, chicken soup,diced cooked potatoes, variety of cheese- 46-65. Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. **Repeat Violation**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Beverage Air three door cooler- sausage, beef franks, gravy, ham, cooked spinach, turkey sausage, chicken soup,diced cooked potatoes, variety of cheese- 46-65. Observed raw chicken wings on prep table in the kitchen at 65°F. Operator stored raw chicken wings in reach in cooler. **Corrected On-Site** **Repeat Violation**
41-10-4    High Priority - Toxic substance/chemical improperly stored. Observed can of Flying Insect Killer stored on top of reach in cooler.
16-33-4    Intermediate - Chemical test kit not used to ensure proper sanitization of equipment and utensils when using a chemical sanitizer. Observed sealed packages of chlorine test kits on top of dish machine.
02C-03-5    Intermediate - Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Observed milk, cooked ham, turkey bacon, sausage, opened a few days ago per Operator.
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed food storage shelves soiled with food debris.
31A-11-4    Intermediate - Handwash sink used for purposes other than hand washing. Observed soiled utensils stored in hand wash sink at front counter and employee filling coffee jug at same hand wash sink.
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Kitchen area.
53B-01-5    Intermediate - No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. **Repeat Violation**
31B-03-4    Intermediate - No soap provided at handwash sink. Kitchen area.
41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Observed degreaser bottle without a label.
