Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

32-12-5    Basic - Covered waste receptacle not provided in women's bathroom. **Warning**

35A-03-4    Basic - Dead roaches on premises. Observed 1 dead roach underneath the shelf above reach in cooler located in front of the cook line. **Warning**
12B-02-4    Basic - Employee eating in a food preparation or other restricted area. Observed a water bottle on the prep table and the employee drinking in the cook line. **Warning**
08B-49-4    Basic - Employee personal food not properly identified and segregated from food to be served to the public. Observed employee soda cans over food in walk in cooler. Observed raw duck in the kitchen area. **Warning**

10-17-4    Basic - In-use knife/knives stored in cracks between pieces of equipment. Observed over the triple sink. **Warning**
36-62-4    Basic - Light not functioning. Observed 3 lights completely out in cookline area **Warning**

05-09-4    Basic - No conspicuously located ambient air temperature thermometer in holding unit. Observed in the reach in cooler located next to the steam table. **Warning**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed the walk in gasket soiled. Observed the walk in cooler fan guards soiled. Observed the reach in cooler door soiled. **Warning**

12B-13-4    Basic - Opened employee beverage container in a cold holding unit with food to be served to customers. Observed employees can soda in the walk in cooler on top of food. **Warning**
35B-03-4    Basic - Outer openings not protected with self-closing doors. Observed the back door open with a screen door. **Warning**
14-11-4    Basic - Reach in cooler gaskets in poor repair. Observed reach in cooler located in the cook line. **Warning**
21-10-4    Basic - Soiled dry wiping cloth in use. Observed towel on prep table. **Warning**
08B-12-5    Basic - Stored food not covered. Observed pork wanton inside an uncovered container in the reach in freezer. **Warning**
14-17-4    Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface. **Warning**
08B-27-4    High Priority - Food placed in soiled container/equipment. Observed buckets soiled with rice sugar and flour. **Warning**
22-45-4    High Priority - Food-contact surfaces not sanitized after cleaning, before use. Do not use equipment/utensils not properly sanitized. Observed employee washing dishes with out using sanitizer. **Warning**
14-15-4    High Priority - Nonfood-grade containers used for food storage - direct contact with food. Observed a plastic gallon with the bottom cut off use to scoop rice. Also observed cardboard boxes used a food container. **Warning**
08A-02-6    High Priority - Raw animal food stored over or with ready-to-eat food in a freezer - not all products commercially packaged. Observed raw chicken above pork wanton inside the freezer. **Warning**
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw eggs over packaged soy sauce. **Warning**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed 50 + live roaches between the reach in cooler and the steam table Located in the cook line area. Also observed 10 + live roaches around 3 compartment sink crawling on the shelves. **Warning**
01B-14-4    High Priority - Shell eggs in use or stored with cracks or broken shells. See stop sale. Observed an egg crack in the walk in cooler. The operator discarded it. **Corrective Action Taken** **Warning**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed bean sprouts (80 F - Cold Holding); garlic in oil (86 F - Cold Holding) just set in the kitchen close to the cook line ( less than 4 hrs as per operator). Operator placed the food inside the reach in cooler **Warning**
53B-15-4    Intermediate - Employee has not received adequate training related to their assigned duties as evidenced by lack of general knowledge in performing their assigned duties. Observed employee washing dishes without completing the sanitizing step. **Warning**
03D-15-4    Intermediate - Food being cooled by nonapproved method as evidenced by inadequate rate of cooling during time of inspection. Observed chicken at 117 f cooling at room temperature in the kitchen, as per operator the chicken was cooked 40 minutes ago. 41-17-4    Intermediate - Spray bottle containing toxic substance not labeled. Observed a spray bottle with a red liquid not labeled. **Warning**
27-16-4    Intermediate - Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at employee handwash sink.
