Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

14-01-5    Basic - Bowl or other container with no handle used to dispense food. Scooping with take out containers. **Repeat Violation**
06-09-1    Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package.
14-11-4    Basic - Equipment in poor repair. Rice por handle broken
14-42-4    Basic - Hood filters not all placed properly in automatic fire suppression/exhaust system.
14-69-4    Basic - Ice buildup in reach-in freezer and/or walk-in freezer. White chest freezer near dishwash machine. **Repeat Violation**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Tops of chest freezers in poor repair, rusted and pitted.
36-30-4    Basic - Wall not smooth, nonabsorbent and easily cleanable in food preparation, food storage, warewashing areas or bathroom. Wall behind rice por with large hole in wall.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. 4 live roaches on wall at handwash sink under paper towels 1 live on wall at rice Steamers.
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Two rodent droppings on dishwasher.
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Sushi rice from previous day used. Advised to discard. Stop sale issued.
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Time as a public health control used for sushi rice. Not discarded and was being used for lunch. Temped at 70-80f, and not marked. Stop sale issued.
02C-03-5    Intermediate - Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening.
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Can opener blade heavily soiled. **Repeat Violation**
