Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

06-08-5    Basic - Time/temperature control for safety food thawed under water that was barely running/dripping. Observed shrimp in handwash sink in standing water. Operator moved faucet over shrimp in three compartment sink. **Warning**
08B-54-4    Basic - Uncovered food stored near sink exposed to splash. Observed flour and corn starch stored in rear kitchen area uncovered. Operator moved top and covered. **Warning**
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed walls soiled with food debris. **Warning**
29-03-4    Basic - Water draining onto floor surface. Observed handwash sink in rear kitchen area without drain. **Warning**
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry. Observed mop stored in mop bucket not dried and clumped together. **Warning**
21-07-4    Basic - Wiping cloth chlorine sanitizing solution not at proper minimum strength. Observed Sanitizer Bucket at Chlorine 0ppm at three compartment sink. **Warning**
12A-12-4    High Priority - Employee switched from working with raw food to cooking without washing hands. Observed employee cutting raw chicken in kitchen, employees moved to cooking in cook line without washing hands. **Warning**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 1 small flying flying by cutting boards in the back where active. **Warning**
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw egg stored above produce in walk-in cooler. Observed chicken being cut above flour containers in rear prep area. **Warning**
08A-17-6    High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. 1) Observed raw steak on stick and raw chicken on stick stored above ready to eat fries, cooked pork, cooked chicken in cook line freezer. 2) Observed raw shrimp stored above ready to eat pork, chicken, krab in rear freezer. **Warning**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed counter - egg roll (72°F - Cold Holding); breaded chicken (65°F - Cold Holding), per operator cooked previously and then stored in walk-in cooler. Removed this morning and stored on counter. Item rose above 41°F. Operator removed and stored chicken in cooler and discarded egg rolls. **Corrected On-Site** **Warning**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed walk-in cooler shelves, containers soiled with food debris. **Warning**
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Observed handwash sink in rear kitchen area and bathroom blocked by various items such as boxes, table, wires, hose, mop bucket. Operator cleared items away from sinks. **Corrected On-Site** **Warning**
16-37-1    Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. **Warning**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Observed no paper towel at handwash in rear kitchen area. Suggested to operator to provide soap. **Warning**
05-08-4    Intermediate - No probe thermometer provided to measure temperature of food products. **Warning**

02C-04-5    Intermediate - Operator is not properly tracking/marking the number of days ready-to-eat, time/temperature control for safety food prepared onsite was held at refrigeration temperatures prior to freezing in order to properly date mark the food when it is thawed and held at refrigeration temperatures again. Observed multiple items in reach in coolers, per operator fully cooked and stored previously. No date marking indicating date of preparation. **Warning**
02C-02-5    Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed chicken per operator cooked yesterday and earlier. No date mark in place. **Warning**
