Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

06-09-1    Basic - Commercially processed reduced oxygen packaged fish bearing a label indicating that it is to remain frozen until time of use no longer frozen and not removed from reduced oxygen package. In reach in cooler at server station, tuna label bears warning that package must be opened prior to thawing. Also, pan of shrimp thawing in standing water and fish thawing in standing water in sink.
12B-07-4    Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. One drink on cutting board on make table at cookline. One drink next to rice holder. Both were moved. **Corrected On-Site** **Repeat Violation**

14-11-4    Basic - Equipment in poor repair. Reach in cooler on cookline has torn gaskets. Ice build up in reach in chest freezer. Also, pans with no handles being used as scoops in rice and potatoes. **Repeat Violation**
10-14-5    Basic - Ice bucket/shovel stored improperly between uses. Two buckets on top of ice machine, not inverted. Also, knife between cooler and prep table on cookline. Manager moved knife. **Corrective Action Taken** **Repeat Violation**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Shelves under sushi bar soiled. **Repeat Violation**
36-50-4    Basic - Unclean building components, attachments or fixtures. Floor is soiled in kitchen under equipment. Ceiling in kitchen is dusty near vents. **Repeat Violation**
29-11-4    Basic - Water leaking from pipe and/or faucet/handle. Under triple sink, water leaking from pipe

12A-29-4    High Priority - Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. 08A-15-5    High Priority - Raw animal food dripping into container of ready-to-eat food and/or raw animal food stored over ready to eat foods. See stop sale. In walk in freezer, tray of raw beef dripping blood onto tray of zucchini. Stop sale issued. Also, pans of raw chicken and shrimp stored on shelves over seafood salad and cooked chicken in walk in cooler. Pans of raw beef and stored over zucchini. 01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. In walk in freezer, Tray of zucchini stored under tray of raw beef. Beef blood dripping on to zucchini.
29-42-4    High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet. Splitter added with no device on connection side of hose. **Repeat Violation** **Admin Complaint**
41-27-4    High Priority - Wiping cloth sanitizer solution exceeds the maximum concentration allowed. In bucket in server station: chlorine sanitizer 200+ ppm. Also, two spray bottles with bleach and degreaser in ware wash area are unlabeled. **Repeat Violation** **Admin Complaint**
31A-11-4    Intermediate - Handwash sink used for purposes other than handwashing. Female employee dumped sanitizer in sink in server station and began adding water to bucket. Also, two sinks in kitchen had scrub pads inside. Scrub pads were removed. **Corrective Action Taken**
