Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

23-24-4    Basic - Buildup of food debris/soil residue on equipment door handles. Observed heavy food debris buildup on the grip area of the handles on the refrigerated make table on the cook line. **Warning**
36-32-5    Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed cracks in the ceiling over the old Mongolian buffet unit. **Warning**
35A-03-4    Basic - Dead roaches on premises. Observed 5 dead cockroaches on the floor between to the wall and the side of the large ice machine at the server beverage area. Operator cleaned away the dead vermin. On going self-inspection, treatment, and cleaning is needed to ensure that vermin are not seen during inspections. **Corrective Action Taken** **Warning**
35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. Observed large gaps around the door to the dry storage shed when the door is closed. Food is stored in the shed. **Warning**
36-22-4    Basic - Floor area(s) covered with standing water. Observed standing water on the floor throughout the back kitchen due to numerous drain leaks **Warning**
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed old dried food debris on the floor under shelving and storage racks in both the walk-in refrigerator and freezer, under the cook line equipment and under the dish washer. Observed food and debris buildup on the floor of the dry storage shed **Warning**
36-17-5    Basic - Floor tiles missing and/or in disrepair. Observed the floor tiles cracked and broken away in the walk-in freezer **Warning**
10-07-4    Basic - In-use utensil stored in standing water less than 135 degrees Fahrenheit. Observed the ice cream scoops stored In standing water at 73F. **Warning**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed old dried food debris on the shelves in the walk-in refrigerator. **Warning**
25-05-4    Basic - Single-service articles improperly stored. Observed boxes of straws and food containers stored on the floor of the Mongolian grill area **Warning**
08B-12-5    Basic - Stored food not covered. Observed an open bag of rice in the exterior dry storage shed. **Warning**
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed grease and debris buildup on the walls and doors of the walk-in refrigerator and freezer **Warning**
29-03-4    Basic - Water draining onto floor surface. Observed the drain leaking under the triple sink and running onto the floor **Warning**
29-11-4    Basic - Water leaking from pipe and/or faucet/handle. Observed the faucet leaks when shut off on the hand washing sink by the walk-in refrigerator. **Warning**
03D-01-5    High Priority - Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 70 degrees Fahrenheit within 2 hours. Observed fried rice prepared 4/19 (45F - Cold Holding) in the reach in refrigerator by the convection oven. The cold held food measures 42F in the same refrigerator. 08B-27-4    High Priority - Food placed in soiled container/equipment. Observed cut broccoli crowns returned to the original shipping box instead of placing them in a cleaned and sanitized container. **Warning**
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed a package of raw chicken hearts on the shelf over cooked lo mein noodle in the walk-in refrigerator. **Warning**
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed fried rice prepared 4/19 (45F - Cold Holding) in the reach in refrigerator by the convection oven. The cold held food measures 42F in the same refrigerator. 22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed food debris buildup on the exterior of the ingredient containers on the shelf on the back wall. **Warning**
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Observed a bucket on the hand washing sink by the walk-in refrigerator. The bucket was removed to create access for hand washing. **Corrected On-Site** **Warning**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. Both hand washing sinks lack paper towels in the kitchen. The operator provided paper towels upon request. **Corrected On-Site** **Warning**

53B-05-5    Intermediate - Required employee training expired for all employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Observe done employee present that has worked more than 60 days whose food handler training expired in February 2022. Remaining employees present have only been employed for 2 weeks. **Warning**
29-28-4    Intermediate - Water treatment device has not been inspected or serviced according to manufacturer's instructions. Observed the water filter behind the ice machine not marked with a date of either installation or 1 year expiration date after installation. **Warning**
27-16-4    Intermediate - Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at employee handwash sink. Observed only 77F water available at the hand washing sink by the triple sink. The hot water was shut off under the sink. The hot water was turned on and the water measured 110F. **Corrected On-Site** **Warning**
