Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-32-5    Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed approximately 5 ceiling tiles in the main kitchen in disrepair and observed peeling paint inside the walk in cooler adjacent to the dish washer. 14-11-4    Basic - Equipment in poor repair. Observed walk in cooler gasket in disrepair located next to server station area. Operator stated they have service scheduled to fix all gaskets throughout the kitchen. **Corrective Action Taken** **Warning**
36-63-4    Basic - Exhaust/ventilation hood system not designed to prevent grease or condensation from dripping onto food, clean equipment and utensils, and single-service items located above cookline. 35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside located in the rear of the kitchen near walk in freezer. As a repeat violation the importance of following proper protocol with this violation was reinforced with the operator. **Repeat Violation** **Warning**
36-22-4    Basic - Floor area(s) covered with standing water in walk in cooler next to office. **Warning**
36-73-4    Basic - Floor soiled/has accumulation of debris located under cookline. Observed accumulation of debris behind server station console. **Warning**
36-17-5    Basic - Floor tiles missing and/or in disrepair and/or in disrepair throughout kitchen. **Warning**

36-24-5    Basic - Hole in or other damage to wall. Observed an approximately 2 inch by 2 inch hole in wall under breading prep table. Observed approximate 12 inch by 2 inch area in disrepair at the corner near the door near the dressings table. **Warning**
33-31-5    Basic - Unnecessary items/unused equipment on the premises. Observed construction materials on the rear side of the building as well as next to the trash compactor. **Warning**
14-17-4    Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface located in both walk in coolers. As a repeat violation the importance of following proper protocol with this violation was reinforced with the operator. **Repeat Violation** **Warning**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Observed approximately 11 rodent droppings under the dishwasher. Observed 3 rodent droppings next to HVAC unit across from the walk in freezer. Observed 2 rodent droppings under dry storage shelves next to ice machine. **Warning**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed butter at 52°F in container of ice on food line next to steam table. Operator stated they were there for less than 2 hours. Operator removed from container and put into freezer to rapid cool. 41-10-4    High Priority - Toxic substance/chemical improperly stored. Observed can of paint stored next to drink containers in the liquor room. Operator removed and stored properly. **Corrected On-Site** **Warning**
