A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Bathroom facility in disrepair.
Toilet in men's bathroom does not flush and per phone call with operator they are working on having it fixed. **Warning**
Basic - Dead roaches on premises.
Approximately 12 dead roaches under triple sink and water heater by walk in cooler in preparation area.
Approximately 20 dead roaches under yogurt dispenser machines in prep area.
Two dead roaches in hallway in front of men's bathroom. **Warning**
Basic - Food stored on floor.
Three boxes of candy yogurt toppings under table by walk in cooler, employee corrected during inspection. **Corrected On-Site**
High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area.
Approximately 10 flying insects around yogurt dispenser machines in preparation area. **Repeat Violation** **Admin Complaint**
High Priority - Roach activity present as evidenced by live roaches found.
Approximately 12 live roaches around water heater in preparation area.
Approximately 6 live roaches under yogurt dispenser machines in prep area. **Repeat Violation** **Admin Complaint**
High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit.
whipped cream (49F - Cold Holding) in can on buffet line sitting above fill line, per employee can was taken out of walk in cooler approximately three hours ago, can placed back in walk in cooler. **Corrective Action Taken** **Repeat Violation** **Admin Complaint**
Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime.
Underside of two yogurt machines around nozzles in dining area have a build up of debris. **Repeat Violation**
Intermediate - No paper towels or mechanical hand drying device provided at handwash sink.
Both hand wash stations in preparation area, employee found paper towels for one hand wash station. **Corrective Action Taken** **Repeat Violation**
Intermediate - No proof of required state approved employee training provided for any employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233.
Employee on site is not able to locate any proof of training for any employees hired over 60 days. **Warning**
Intermediate - Spray bottle containing toxic substance not labeled.

Intermediate - Water with a temperature of at least 100 degrees Fahrenheit not provided/shut off at employee handwash sink.
Hot water shut off at hand wash station in preparation area by door leading to bathrooms and at hand wash station in men's bathroom, hot water turned back on. **Corrected On-Site** **Repeat Violation**
