Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

25-05-4    Basic - Single-service articles improperly stored. Observed straws stored on floor in upstairs storage. Operator moved straws off of floor. **Corrected On-Site**
29-20-5    Basic - Standing water or very slow draining water in handwash sink, three-compartment sink, or mop sink. Observed handwash sink at bar area draining slowly.
25-02-4    Basic - Unwrapped single-service utensils not presented so that only the handles are touched. Observed unwrapped straws at bar counter.
08B-56-4    High Priority - Food stored in ice used for drinks. See stop sale. Observed employee drink in ice machine in ware wash area. Operator removed drink.
01D-01-5    High Priority - Nonexempt fish offered raw or undercooked has not undergone proper parasite destruction. Fish must be fully cooked or discarded. Observed raw salmon tartare with no consumer advisory.
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed raw chicken and raw beef over shredded cheese and sliced cheese in reach in cooler next to reach in freezer next to ware washing area. Operator moved items to be properly stored. **Corrected On-Site**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed 1 small crawling insect at front counter area inside and outside of reach in cooler under spices crawling on covered croutons and plates. Observed approximately 4 small crawling insects crawling on walls and floor around ice machine in ware washing area not touching food or food contact surfaces.
03A-03-5    High Priority - Shell eggs not stored at an ambient air temperature of 45 degrees Fahrenheit or less. Observed raw shell eggs stored at front counter sitting at room temperature,as per operator eggs were out for approximately one hour. Operator moved eggs to reach in cooler. **Corrected On-Site**
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Observed 1 small crawling insect at front counter area inside and outside of reach in cooler under spices crawling on covered croutons and plates.
01C-03-4    Intermediate - Clam/mussel/oyster tags not marked with last date served.
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed can opener at front counter area soiled
05-08-4    Intermediate - No probe thermometer provided to measure temperature of food products.
