Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

12B-07-4    Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. Observed an employee drink with tea on the prep table in kitchen , no lid , no straw. Operator moved drink away from food areas. **Warning**
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Observed a cellphone stored on a serving plate at shelving across salad flip top. Operator removed. **Corrected On-Site** **Warning**

02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed 2 squeeze bottles of dark liquid unlabeled. Observed 3 metal pans all containing a similar looking granules. Operator labeled as soy sauce, salt and sugar. **Corrected On-Site** **Warning**
12A-20-4    High Priority - Employee washed hands with no soap and gloves on. Instructed employee to wash hands properly. **Warning**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. 2 flies observed flying around and landing on unwashed limes, flip-top lid, and inverted to go containers along the cook line. Operator killed both and sanitized area. **Warning**
08A-14-5    High Priority - Raw animal food not separated from ready-to-eat food during preparation. Observed in the 2 door reach in freezer located by office door raw chicken over fries. Observed frozen seafood over frozen vegetables. **Warning**
08A-05-6    High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Observed in the2 door lowboy across from the fryer a plate of raw chicken above open containers of ready to eat items. Operator rearranged to move chicken below. **Corrected On-Site** **Warning**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. 1 live roach observed in the back of kitchen on the doorframe of office door located inside kitchen. 1 live roach on floor by walk-in cooler door. 03A-03-5    High Priority - Shell eggs not stored at an ambient air temperature of 45 degrees Fahrenheit or less. Observed two flats of shell eggs on top of microwave on cook line. 03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Flip top across from fryer: raw shrimp (56°F - Cold Holding); cooked shrimp (50°F - Cold Holding); raw chicken (42°F-56°F - Cold Holding); cooked chicken cubes (56°F - Cold Holding) all foods in unit less than 4 hours. 53A-05-6    Intermediate - No currently certified food service manager on duty with four or more employees engaged in food preparation/handling.
