Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

16-13-5    Basic - Equipment and utensils not washed, rinsed and sanitized in the correct order in three-compartment sink. Do not use dishes/equipment not properly sanitized. Reviewed proper procedures and setup the sink correctly. **Warning**
14-11-4    Basic - Equipment in poor repair. Observed in the middle storage room on the White refrigerator, the bottom gasket is completely separated from the door. **Warning**
35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. At the rear kitchen door, there is a gap on the entire perimeter. **Warning**
08B-38-4    Basic - Food stored on floor. Observed in the walk-in cooler, chicken in a bucket and onions stored on the floor. The employee moved items to shelves. **Warning**
33-23-4    Basic - Grease receptacle not on proper pad/nonabsorbent pad. Observed the grease receptacle halfway on asphalt and dirt. **Repeat Violation** **Warning**
33-16-4    Basic - Open dumpster lid. Employee closed the lid during the inspection. **Corrected On-Site** **Warning**
35B-05-4    Basic - Outer openings not protected during operation and vermin and/or environmental cross contamination present. Observed the kitchen screen door open and the mesh too large. The door does not seal well. **Warning**
35B-13-4    Basic - Screening is not 16-mesh to the inch. Observed on the kitchen screen door. **Warning**
36-62-4    Basic - Two light fixtures not functioning in the dry storage area. Not enough lighting in the room. **Warning**
12A-28-4    High Priority - Employee touched soiled cloth on waist and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee advised to wash and dry hands properly and remove cloth from his waist. **Corrected On-Site** **Warning**
22-45-4    High Priority - Food-contact surfaces not sanitized after cleaning, before use. Do not use equipment/utensils not properly sanitized. Observed pots being put away with no sanitizing step. The pots were set aside to be rewashed. **Warning**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. 1. Observed approximately 10 small flies flying around and landing on clean utensils and storage containers on shelves and the dish machine table. 2. Observed 10 small flies landing on shelving above the dish machine and three-compartment sink. 3. Observed over 20 small flies landing on clean serving cups stored on a shelf in the kitchen. Operator began killing flies during the inspection. **Warning**
35A-09-4    High Priority - Presence of insects. Observed live moths inside a closed storage container of rice. See stop sale. **Warning**
08A-20-5    High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. 1. Observed raw chicken stored behind raw beef in the left flip top cooler on the cook line. The employee rearranged for proper storage. Reviewed proper storage with the employee. 2. 08A-17-6    High Priority - Raw animal foods not properly separated from one another based upon minimum required cooking temperature when stored in a freezer - not all products commercially packaged. Observed repacked chicken stored above repackaged beef and cooked beans in the reach in freezer. The employee rearranged for proper storage. **Corrected On-Site** **Warning**
01B-13-4    High Priority - Stop Sale issued due to food not being in a wholesome, sound condition. Presence of insects. Observed live moths inside a closed storage container of rice. **Warning**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed in the Right flip top cooler; tomatoes (48F - Cold Holding). Per the employee the tomatoes were placed in the cooler yesterday. The employee voluntarily discarded the tomatoes. Butter (50F - Cold Holding) - per the employee the item was stocked in the cooler 2 hours prior. Butter placed in alternate refrigeration to quick chill. **Corrective Action Taken** **Warning**
41-27-4    High Priority - Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Observed the chlorine sanitizer at over 200ppm. The solution was remade at 100ppm. **Warning**
02B-01-5    Intermediate - Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. The establishment serves raw fish ceviche that is not identified as covered by the consumer advisory. An employee began correcting the menus during the inspection. **Corrective Action Taken** **Warning**
16-37-1    Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths. Observed no chlorine test kit available. **Warning**
02C-02-5    Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed cooked yucca in the walk-in cooler not date marked. Per the manager the item was prepared yesterday. The manager added the date mark. **Repeat Violation** **Warning**
