Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-32-5    Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed ceiling with water damage dripping water in the middle of kitchen.
36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling with heavy dust buildup above make table in kitchen.
13-07-4    Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Observed employee with watch on while preparing food in kitchen. Operator had employee remove watch. **Corrected On-Site**
14-11-4    Basic - Equipment in poor repair. Observed gaskets of combo fridge and of lowboys and make table torn.
08B-47-4    Basic - Food not stored at least 6 inches off of the floor. Observed raw hamburger beef stored on floor in walk-in cooler. Operator removed and stored appropriately on shelves. **Corrected On-Site**
38-07-4    Basic - Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. Observed lights in kitchen missing shielding.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed gasket of make tables with food build up. **Repeat Violation**
14-17-4    Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface. Observed rust on shelves in walk-in cooler and in standing cooler in kitchen.
03D-02-5    High Priority - Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within 6 hours. Observed ribs (45F - Cooling); marinara (45F - Cooling), per operator items were cooked previously and stored in cooler since 03/11 and 03/16 respectively. Per operator items stored and not moved from cooler since cooling started. See Stop Sale.
22-43-4    High Priority - Quaternary ammonium sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. Observed Triple Sink (Quaternary 0ppm), operator opted to drain and replace with solution at 200 ppm. **Corrected On-Site**

Observed ribs (45F - Cooling); marinara (45F - Cooling), per operator items were cooked previously and stored in cooler since 03/11 and 03/16 respectively. 01C-05-4    Intermediate - Clam/mussel/oyster tags not maintained in chronological order according to the last date they were served in the establishment. Observed tags not stored in chronological order, tags are stored in plastic container above cook line and not ordered,
01C-03-4    Intermediate - Clam/mussel/oyster tags not marked with last date served. Observed all oyster and clam tags missing date of last sale.
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed soda gun at front counter soiled with slime/mold-like substance.
01C-01-4    Intermediate - No tag on/for original container of raw, in-shell clams/mussels/oysters. Observed two containers of Oysters missing shell tags, operator stated tags were put with maintained tags.
