Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

14-01-5    Basic - Bowl or other container with no handle used to dispense food. Upon inspection observed bowl resting in large flour container back of kitchen. Observed server dip cup to get ice from ice bin.
35A-03-4    Basic - Dead roaches on premises. Upon inspection observed a total of 4 dead roaches in the following areas: 2 dead roaches in a drain below hand washing sink back right side of kitchen, 1 dead roach under oven, 1 dead roach under dry storage rack right side of cook line. **Warning**
16-02-4    Basic - Dishmachine missing baffles, curtains in wash/rinse tanks. Dish Machine curtain in disrepair.
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Multiple areas in kitchen prep with jackets, purse stored next to clean equipment. Cell phone on top prep next to clean dishes in small cook line. Operator removed items of concern during inspection. **Corrected On-Site**
24-08-4    Basic - Equipment and utensils not properly air-dried - wet nesting. Upon inspection observed stacked drinking cups with moisture in between in wait station area.
14-69-4    Basic - Ice buildup in reach-in freezer. Upon inspection observed ice buildup in bottom of reach-in freezer back of kitchen.
10-20-4    Basic - In-use tongs stored on equipment door handle between uses. Upon inspection observed 2 pairs of tongs on oven handle on back up kitchen. Operator removed tonges during inspection. **Corrected On-Site**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, Upon inspection hood system with grease buildup.
29-49-6    Basic - Standing water in bottom of reach-in-cooler. Upon inspection observed standing water in multiple reach-in coolers.
14-15-4    High Priority - Nonfood-grade containers used for food storage - direct contact with food. Upon inspection observed ice in retail bag used to cool down food item in reach in cooler back of kitchen. Operator corrected storage during inspection. **Corrected On-Site**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Upon inspection observed 1 live roach on floor in front of small salad prep cooler. Operator exterminated and cleaned/sanitized area. **Corrective Action Taken** **Warning**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Small salad prep cooler right side of cook line cut lettuce 45F, shrimp 45F. Door opened frequently and operator removed and placed in colder unit. **Corrective Action Taken**
16-37-1    Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths.
31B-03-4    Intermediate - No soap provided at handwash sink. Located right side of cook line near dry equipment storage.
