Violations:
A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Cutting board has cut marks and is no longer cleanable
Several cutting boards on the cook line. Soiled cutting board at bar.
As a repeat violation the importance of following proper protocol with this violation was reinforced with the employee. **Repeat Violation** **Warning**
Basic - Ice scoop handle in contact with ice. Observed two scoops stored with handle in ice bins at bar. Educated employees. Employees removed scoops from ice. **Corrected On-Site** **Warning**
Basic - Raw fruits/vegetables not washed prior to preparation.
Employee cut fresh mushrooms before washing.
Chef discarded the mushrooms. **Corrective Action Taken** **Warning**
Basic - Water leaking from pipe and/or faucet/handle. Water leaking, all over bar area. Per employee, water coming from espresso machine due to broken line. Service was called. Employee cleaned and sanitized area, but the water has filled the back corner of the bar area again during this inspection. **Warning**
High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food.
Employee handled raw fish change gloves then handled clean utensils without washing hands.
Instructed employee and manager on proper hand washing protocol. **Warning**
High Priority - Employee rubbed hands together for less than 10-15 seconds while washing hands.Observed employee at salad preparation wash hands for less than 15 seconds.

High Priority - Employee washed hands with cold water. Observed employee leave employee bathroom. Inspector went behind her to conduct bathroom check. When hot water was turned on, only cold water came out. Hot water did get to 114 within about 10 seconds. Educated operator regarding use of 100 degree hot water for hand wash. **Warning**
High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed six flying insects at bar area flying around. **Warning**
High Priority - Raw animal food stored over or with unwashed produce. Observed at walk-in cooler, unwashed cucumber over Romano in walk-in cooler. **Warning**
High Priority - Stop Sale issued due to food not being in a wholesome, sound condition.

Rioja (93F - Reheating) stored in the steam table on the cook line. Chef stated the item had been reheating more then 2 hours.

High Priority - Time/temperature control for safety food reheated for hot holding not reaching 165 degrees Fahrenheit for 15 seconds within 2 hours.

Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed employee put cleaning bucket from floor onto cutting board being used at bar. Observed soda gun at south side of bar resting on floor at bar area. Observed soiled soda gun at north side of bar area. Educated employee. **Warning**
Intermediate - Handwash sink used for purposes other than handwashing. Hand washing sink had ice and small pans in it, at bar. Educated employee. Employee removed items from sink. **Warning**
