Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-03-4    Basic - Dead roaches on premises. 1. Observed 2 dead roaches on the floor behind a prep table on the cook line next to a chest freezer in the kitchen. 2. Observed 2 dead roaches on the floor behind a prep table at the end of the cook line in the kitchen. Employee removed the roaches and cleaned and sanitized the areas. **Warning**
14-11-4    Basic - Equipment in poor repair. 1. Observed the lower left drawer gasket on the middle line flip top cooler in poor repair. 2. Observed the door to the walk-in freezer does not shut tightly. Observed water dripping on the floor from the condensation pan in the walk-in freezer. **Warning**
36-73-4    Basic - Floor soiled/has accumulation of debris around the perimeter of the kitchen and storage area at the cove base. **Repeat Violation** **Warning**
36-24-5    Basic - Hole in or other damage to wall above the prep sink near the mop sink. **Warning**
27-14-4    Basic - Self-closing metering faucet does not provide water for at least 15 seconds at hand wash sink. The faucet must be held in order for water to run continuously. **Repeat Violation** **Warning**
12A-17-4    High Priority - Due to self metering device in handwash sink, Employee rubbed hands together for less than 10-15 seconds while washing hands. Instructed employee to wash hand again properly and observed employee do so. **Corrected On-Site** **Repeat Violation** **Admin Complaint**
12A-07-5    High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Observed dishwasher leave soiled dish side of the dishwasher and remove gloves and replace before moving to clean side of the dishwasher. 03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. 1. Observed in the wait station in a built in cold table, sliced tomatoes (46F - Cold Holding); leaf lettuce (61F - Cold Holding). Per the manager the items were place in the table approximately 3 hours prior. Items moved to alternate refrigeration to quick-chill. Recommended TPHC and provided form. 2. Observed in the right side line flip top cooler, blue cheese (52F - Cold Holding); cooked mushrooms (46F - Cooling @ 10:25 for 30 minutes); feta cheese (48F - Cold Holding); liquid eggs (55F - Cold Holding); French toast batter (54F - Cold Holding). Per the employee all items stocked approximately 3 hours prior. All items moved to walk-in cooler to quick chill or placed in ice baths. Retested liquid eggs at 42°F and French toast batter at 41°F. 3. Observed in the lower portion of the middle flip top cooler, ground beef on lower left side, (48F - Cold Holding). Per the employee the item was stocked 3 hours prior. Item moved to walk-in cooler to quick chill. 4. Observed in the wait station Glass door cooler; butter (51F - Cold Holding). Observed an excessive quantity stocked high in the pan. Recommended stocking a smaller quantity. Item moved to alternate refrigeration to quick chill. **Corrective Action Taken** **Warning**
03B-01-6    High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed in the steam table, sausage links (117-125F - Hot Holding). Per the employee the item was cooked 3 hours prior. Item placed on flat top grill to reheat. **Corrective Action Taken** **Warning**
12A-03-4    Intermediate - Employee washed hands in a sink other than an approved handwash sink. Dishwasher washed hands in the three compartment sink. Instructed employee to wash hands in the hand wash sink and observed employee doing so. **Corrected On-Site** **Warning**
