Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-06-4    Basic - Accumulation of dead or trapped birds, insects, rodents, or other pests, in control devices. Observed 6 sticky fly traps throughout kitchen area with over 50 per device or more dead flies attached to each device. Operator discarded devices from kitchen area. **Warning**
23-24-4    Basic - Buildup of food debris/soil residue on equipment door handles. Observed build up food debris on equipment door handles and reach in gaskets throughout. **Warning**
36-36-4    Basic - Ceiling tile missing. Observed ceiling tiles missing in dry storage area. **Warning**
35A-03-4    Basic - Dead roaches on premises. Observed approximately 10 roaches on floor at in door bar area under equipment. Observed. Approximately ten or more dead roaches throughout dining room area. Under booths Observed 2 dead roaches at desert area under equipment Observed 1 dead roach inside standing reach in cooler stored with cooked rice and prepped beef kabob in kitchen area. Operator I cleaned and sanitized areas and removed dead roaches. **Warning**

40-06-5    Basic - Observed Employee personal items cellular phones stored in or above a food preparation areas in kitchen area.food, clean equipment and utensils, or single-service items. **Warning**
25-06-4    Basic - Observed Single-service to go containers throughout kitchen area not stored inverted or protected from contamination. **Warning**
02D-01-5    Basic - Observed Working containers of salt, and baking soda on condiment shelf located at prep area removed from original container not identified by common name. **Warning**

14-86-1    High Priority - Non-food grade paper/paper towel used as liner for food container. Observed salt stored in a non food grade container, which was previously a chemical detergent container at dry storage area. **Warning**
08A-20-5    High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Observed raw salmon skewers stored cooked gyros in standing reach in freezer. **Warning**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. Observed approximately 10 or more rodent droppings under soda boxes and CO2 tanks in dry storage area located beside walk in freezer and shelves with condiments. Operator washed floors and removed rodent droppings from surrounding area. **Warning**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Observed cutting board, soda water nozzles at bar area with mold like substance.. Observed soda nozzles of soda machine located in dining area with build up mold like substance. Observed prep sink at kitchen area soiled with grease build up . **Warning**
31A-11-4    Intermediate - Handwash sink used for purposes other than handwashing. Observed hand washing sink located at prep area used as a dump sink for draining water from cooked beef. **Warning**
02C-02-5    Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed rice in standing reach in cooler cooked from Sunday per operator not labeled. Observed lentil cooked from Tuesday not labeled. **Warning**
