Violations:
A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin.
Observed main ice machine to be soiled on the interior of unit, and behind splash guard.
Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance.
Observed AC vents in food dry storage area to be soiled.
Basic - Cutting board has cut marks and is no longer cleanable.
Observed cutting boards on both ends of kitchen on cook line to be grooved and stained.
Basic - Equipment and utensils not properly air-dried - wet nesting.
Observed dishes on clean drying rack in back room next to 3 compartment sink wet nesting.
Basic - Food stored in a location that is exposed to splash/dust.
Observed tray of uncovered raw beef stored directly under drain lines of refrigeration unit, and under soiled /dusty refrigeration fan in walk in cooler.
Manager placed empty pan on top of raw beef in walk-in cooler. **Corrective Action Taken**
Basic - Unwashed fruits/vegetables stored with ready-to-eat food.
Observed unwashed kale stored on the top shelf in walk-in cooler above ready to eat sauces.
High Priority - Employee touched soiled apron/clothes and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands.
Observed grill cook handle raw beef and proceeded to prepare food. Manager spoke to employee and he removed gloves and washed hands properly

Manager spoke to employee and employee changed gloves and washed hands properly. **Corrective Action Taken**
High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area.
Observed one flying insect on cook line near grill area.
Observed one flying insect in dry storage area. **Repeat Violation** **Admin Complaint**
High Priority - Vacuum breaker missing at mop sink faucet or on fitting/splitter added to mop sink faucet.
Observed splitter on mop sink to be missing a vacuum breaker after splitter before green hose connection.
Manager removed hose and stated they will not use. **Corrective Action Taken**
Intermediate - Handwash sink not accessible for employee use at all times.
Observed Hand sink at entrance of kitchen to be blocked with a metal bowl inside sink.
Manager removed and stored properly. **Corrected On-Site**
