Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

02D-03-4    Basic - Label on a food item prepared and packaged onsite for customer self-service does not include product identity/description, date product was packaged and name and address of the establishment that prepared and packaged the food. Cupcakes in self service cooler purchased from Cupcake Connection does not have required information.

14-47-4    Basic - Nonfood-contact equipment exposed to splash/spillage not constructed of materials that are corrosion resistant, non-absorbent, smooth and easily cleanable. Wood crate under microwave at front counter is not sealed. Multiple shelves under prep prep tables are showing rust. Exterior of walk in freezer showing rust. 23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Orange floor fan in front counter area. Shelves in kitchen and front counter area. Handles for coolers and freezers. Hood filters above fryers in kitchen. **Repeat Violation**
33-16-4    Basic - Open dumpster/grease receptacle lid. Both outside dumpster and grease receptacle.
35B-03-4    Basic - Outer openings not protected with self-closing doors. Rear exit door.
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Two live flies observed in kitchen. Flies killed and discarded. **Corrected On-Site**
41-10-4    High Priority - Toxic substance/chemical improperly stored. Spray bottle containing blue chemical cleaner stored on front counter area with single use items. Bottle moved and stored correctly. **Corrected On-Site**

51-16-7    Intermediate - No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days.
