Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

05-09-4    Basic - No conspicuously located ambient air temperature thermometer in holding unit. Observed at reach in cooler at cook line. **Warning**
16-46-4    Basic - Old labels stuck to food containers after cleaning. Observed at storage shelves at ware washing area. **Warning**

21-07-4    Basic - Wiping cloth chlorine sanitizing solution not at proper minimum strength. Observed chlorine solution at 0ppm. Employee took container to replenish. **Corrective Action Taken** **Warning**
22-41-4    High Priority - Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed chlorine machine at 0ppm. **Warning**
12A-16-4    High Priority - Employee began working with food, handling clean equipment or utensils, or touching unwrapped single-service items without first washing hands. Observed at front counter area. **Warning**
03G-05-5    High Priority - Fish packaged in the establishment using a reduced oxygen method not bearing a label indicating that it is to be kept frozen until time of use. Observed salmon at walk in cooler. As per manager, item was vacuum packaged at establishment. **Warning**

Observed raw shrimp (58F - Cold Holding) ;croquettes (50F - Cold Holding); cream (52F - Cold Holding) at reach in cooler at cook line. As per employee less than 2 hours. 02B-01-5    Intermediate - Menu does not identify which items contain raw or undercooked animal foods covered by the consumer advisory. Observed establishment offers raw tuna, caviar and raw salmon on the menu, items not identified as raw. **Warning**
