A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Dead roaches on premises.
Observed 1 dead roach in milk crate above water dispenser located outside of walk in cooler in kitchen. Operator removed cleaned and sanitized.
Observed approximately 4 dead roaches under water dispenser.

Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Floor under two door stainless freezer next triple sink heavily soiled **Warning**
Basic - Old labels stuck to food containers after cleaning. Observed old labels stuck to in use container holding potatoes in walk cooler. Operator removed **Warning**
High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed 2 live flies at front counter hovering around espresso machine not landing.
Observed approximately 3 live flies on cook line hovering above expo window and landing on waste receptacle located directly next to wares. Also observed 1 landing on cutting board to flip top cooler. **Warning**
High Priority - Nonfood-grade bags used in direct contact with food. Cooked rice and raw chicken stored in handled plastic bag. Discussed with operator **Warning**
High Priority - Raw animal food stored over/not properly separated from ready-to-eat food. Raw shell eggs stored above ready to eat sauces in cook line flip top. Operator reorganized **Corrected On-Site** **Warning**
High Priority - Roach activity present as evidenced by live roaches found. Observed approximately 6 live roaches under water dispenser located outside of walk in cooler in kitchen. Operator started to remove immediately
Observed approximately 6 live roaches under 2 door stainless reach in freezer next to triple sink **Warning**
High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. raw beef (63°F - Cold Holding) being held at room temperature. Operator states item has been out for approximately 2 hours. Employee started to prepare beef **Warning**
