Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling tiles throughout kitchen to be soiled. Observed ceiling vents throughout kitchen to be soiled. **Repeat Violation** **Warning**
24-08-4    Basic - Equipment and utensils not properly air-dried - wet nesting. Observed sauce pans wet nesting being stored on clean storage rack at ware washing area. **Warning**
36-24-5    Basic - Hole in or other damage to wall. Observed hole in wall at sushi bar along the back counter. **Warning**
22-08-4    Basic - Interior of oven/microwave has accumulation of black substance/grease/food debris. Observed microwave on cook line near grill to be soiled. Manager had employee clean **Corrected On-Site** **Warning**
22-41-4    High Priority - Dishmachine chlorine sanitizer not at proper minimum strength. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. Observed Dish machine at 0 ppm. 12A-17-4    High Priority - Employee rubbed hands together for less than 10-15 seconds while washing hands. Employee washed hands and rubbing hands together for 4-5 seconds. 35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. 6 Rodent droppings under Hot water tank near mop sink. 1 Rodent dropping at mop sink area. 6 Rodent droppings under racks in dry food storage area. 1 Rodent dropping under prep table in middle of kitchen. 1 Rodent dropping under prep table on cook line near stove. 14 Rodent droppings at Sushi bar along the wall of counter directly behind sushi making counter behind/under toaster oven.. 1 Rodent dropping under Hibachi grill near bar storage area. 3 Rodent droppings at Beer and Wine storage area. **Warning**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. ambient temperature of shelled eggs (50F - Cold Holding) in walk in cooler. Employee rushed eggs from kitchen counter into walk in cooler upon inspectors arrival.. operator stated he will keep eggs in cooler. **Admin Complaint**
