Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35B-01-4    Basic - Exterior door has a gap at the threshold that opens to the outside. Located on back door. **Repeat Violation**
36-22-4    Basic - Floor area(s) covered with standing water. Throughout the kitchen area.
36-73-4    Basic - Floor soiled/has accumulation of debris. Located inside walk in freezer floor.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed cooler gasket soiled.
22-16-4    Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Interior of salad cooler in the kitchen area.
21-12-4    Basic - Wet wiping cloth not stored in sanitizing solution between uses. Located on top of prep table in the kitchen area. **Corrected On-Site** **Repeat Violation**
03D-02-5    High Priority - Cooked/heated time/temperature control for safety food not cooled from 135 degrees Fahrenheit to 41 degrees Fahrenheit within 6 hours. Observed cooked brown rice (52F - Cooling)stored inside walk in cooler. As per operator cooked brown rice was prepared yesterday morning.
01B-02-5    High Priority - Stop Sale issued on time/temperature control for safety food due to temperature abuse. Observed cooked pasta (58F - Cold Holding)stored inside salad cooler in the kitchen area. As per operator cooked pasta was prepared 3 days before the inspection. Observed raw beef steak (59F - Cold Holding); pooled egg (53F - Cold Holding) stored inside cook line cooler. As per operator raw steak and pooled eggs was prepared yesterday. Observed cooked brown rice (52F - Cooling)stored inside walk in cooler. As per operator cooked brown rice was prepared yesterday morning.
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed cooked pasta (58F - Cold Holding)stored inside salad cooler in the kitchen area. As per operator cooked pasta was prepared 3 days before the inspection. Observed raw beef steak (59F - Cold Holding); pooled egg (53F - Cold Holding) stored inside cook line cooler. As per operator raw steak and pooled eggs was prepared yesterday. Operator discarded it.
03B-01-6    High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. Observed cooked black beans on the steam table at 122F, as per operator has been there approximately 30 minutes, operator returned to the stove and reheated to 185°F. **Corrected On-Site**
41-10-4    High Priority - Toxic substance/chemical improperly stored. Observed bottle of W-40 stored on top of prep table in the kitchen area next to slicer. **Corrected On-Site**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Cutting board soiled.
02C-02-5    Intermediate - Ready-to-eat, time/temperature control for safety food prepared onsite and held more than 24 hours not properly date marked. Observed cooked shredded beef stored inside walk in cooler not date marked. As per operator cooked shredded beef was prepared 3 days before the inspection.
