Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-01-4    Basic - Floor not cleaned when the least amount of food is exposed. Debris of old food, soiled wiping cloth and can spray on floor between chest freezer and upright unit. Floor drains soiled thru location.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Bottom shelve of prep table soiled with old food.
25-06-4    Basic - Single-service articles not stored inverted or protected from contamination. Unwrapped plastic knives, forks and spoons at bar, Are to be inverted. Corrected.
08B-17-4    Basic - Unwashed fruits/vegetables stored with ready-to-eat food. Unwashed mushrooms stored over the ready to eat sauces in reach in cooler unit.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. behind hand wash sink, Behind the 3 compartment sink near AC unit.
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. One(1) live fly in kitchen / food prep area. **Warning**

35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. One (1) live roach, ladies room north bar room., per dist 4 supervisor on duty, next day callback needed. **Warning**
35A-04-4    High Priority - Rodent activity present as evidenced by rodent droppings found. 4 rodents droppings in utility room with mop sink. Next day callback per dist 4 supervisor. **Warning**
12A-09-4    High Priority - Single-use gloves not changed as needed after changing tasks or when damaged or soiled. Observed cook touch raw shaved beef then go to micro to touch bread without removing soiled gloves. Corrected and re-educated.
53A-03-7    Intermediate - Food manager certification expired. Only one certified manage4 for location expired 6/23/2022 Robin Wheeler.
