Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

22-20-5    Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin.
14-01-5    Basic - Bowl or other container with no handle used to dispense food. Observed on the prep cooler for salads. Also observed in the rice at the steam table. The manager removed the items. **Corrected On-Site**
36-37-5    Basic - Ceiling not smooth, nonabsorbent and easily cleanable in food preparation, food storage, or warewashing areas. Observed in the dry storage area and next to the employee restroom.
36-32-5    Basic - Ceiling/ceiling tile shows damage or is in disrepair. Observed water stains and cracks in the ice machine area.
36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed above the ice machine.
14-09-4    Basic - Cutting board has cut marks and is no longer cleanable. Observed on the prep coolers in the cook line area.
36-24-5    Basic - Hole in or other damage to wall. Observed a hole in the wall behind the mop sink.
10-20-4    Basic - In-use tongs stored on equipment door handle between uses. Observed 3 tongs on the oven door. The manager moved the tongs. **Corrected On-Site**
10-01-5    Basic - In-use utensil in non-time/temperature control for safety food not stored with handle above top of food within a closed container. Observed the handle in the flour. The manager moved the handle. **Corrected On-Site**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Observed the hood filters soiled.
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Observed a large container with tortillas and another with sugar. The manager labeled the containers. **Corrected On-Site**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Observed Fish (49F - Cold Holding); raw shrimp (49F - Cold Holding); raw fish (49F - Cold Holding). As per the manager the items were placed on line less than an hour ago. The manager moved them to the walk in freezer for quick chill. **Corrective Action Taken**
29-34-4    High Priority - Vacuum breaker missing at hose bibb or on fitting/splitter added to hose bibb. Observed next to the door.
02A-02-4    Intermediate - Raw or undercooked oysters offered and consumer advisory provided does not contain the required verbiage.
