A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Single-service articles improperly stored.
Multiple boxes of single service items on floor throughout kitchen and bar areas, employees began to move boxes during inspection. **Corrective Action Take **Corrective Action Taken**
High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area.
Approximately 4 live flying insects around hand wash stations at bar near escalators/stairs.
High Priority - Rodent activity present as evidenced by rodent droppings found.
Approximately 3 rodent droppings behind grease trap under triple sink in corner.
Approximately 20 rodent droppings under soda machines under front counter near main entrance.
Approximately 4 rodent droppings around water heater at inside bar area.
Approximately 5 rodent droppings at bar area by stairs under back counter closest to escalators. **Admin Complaint**
Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths.
No quaternary ammonium test strips.
Intermediate - Written procedures available for use of time as a public health control to hold time/temperature control for safety food incorrect/missing information.
All food items held on time are not on written procedures, operator began to fill in during inspection. **Corrective Action Taken**
