High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Dead roaches on premises.
Observe approximately 2 dead roaches by walk-in refrigerator, 3 dead under hand wash sink at front counter, **Warning**
High Priority - Employee used the bathroom and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands upon returning to employee area.
Observe employee returning from bathroom with gloves on and start food preparation.
Employee stated that they washed hands in bathroom because there is no water in both hand wash sinks. **Warning**
High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area.
Observe 1 flying insect landing on Turkey on prep table. **Warning**
High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature.
Observe raw shelled eggs above raw fish. **Warning**
High Priority - Roach activity present as evidenced by live roaches found.
Observe approximately 1 roach on prep table with food on table ,2 by walk-in refrigerator, 3 in case of water,3 on top plastic container lid, 4 behind 3 compartment sink in kitchen.**Warning** **Warning**
High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit.
2)Observe at prep table rice (65F - Cold Holding); Turkey (59F - Cold Holding); pork (50F - Cold Holding).
Operator stated that they leave them on table till they finish,with no time as public health control for them.
2) Observe at Flip top refrigerator ; cut lettuce,cut tomato (45F - Cold Holding) due to no insert on top just large bowl that doesn't cover the refrigerator opening and cold air is escaping. **Warning**
High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit.
Observe at hot box black rice (127F - Hot Holding). **Warning**
Intermediate - Handwash sink does not have enough water pressure to properly wash hands.
Due to both sinks are lacking hot and cold running water. **Warning**
Intermediate - Handwash sink not accessible for employee use at all times.
Observe lighter , and bleach in hand wash sink in kitchen.
