Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

25-06-4    Basic - Single-service articles not stored inverted or protected from contamination. Silver to go tins near pass through window. Tins inverted. **Corrected On-Site**
29-49-6    Basic - Standing water in bottom of reach-in-cooler. Keg cooler in bar area.
29-20-5    Basic - Standing water or very slow draining water in handwash sink, three-compartment sink, or mop sink. Hand washing sink near walk in cooler. Drain cleared by manager. **Corrected On-Site**
22-42-4    High Priority - Chlorine sanitizer not at proper minimum strength for manual warewashing. Do not use equipment/utensils not properly sanitized. At triple sink, chlorine sanitizer 0ppm. Manager remade solution. 100ppm. **Corrected On-Site**
22-49-4    High Priority - Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly. High temperature dish machine 168F rinse temperature. **Repeat Violation** **Admin Complaint**
12A-07-5    High Priority - Employee failed to wash hands before putting on gloves to initiate a task working with food. Cook in kitchen handled soiled pot, and put gloves on without washing hands beforehand. Manager coached employee on proper hand washing procedures. **Warning**
12A-29-4    High Priority - Employee touched soiled surface and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Cook handled soiled dishes at triple sink and then handled raw beef to cut. See stop sale. **Warning**
12A-20-4    High Priority - Employee washed hands with no soap. Cook on cook line rinsed hands with spray hose in dish and went back to working with food. Manager coached employee on proper hand washing procedures. **Corrective Action Taken** **Warning**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed two live flies in kitchen near dish area. **Warning**
08A-20-5    High Priority - Raw animal foods not properly separated from each other in holding unit based upon minimum required cooking temperature. Raw cut chicken stored over raw cut beef in cooler drawers on cook line. Items moved and stored correctly. **Corrected On-Site**
28-13-5    High Priority - Raw sewage on ground of establishment. Hand washing sink in between dish machine and ice machine in kitchen is backed up. Drain pipe from sink into ground is leaking water onto kitchen floor where inspector has observed several employees tracking through water. Inspector notes strong odor present in area. Establishment has one other hand washing sink on the other side of the kitchen. **Warning**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed four live roaches in server station where drinks and salsa is prepared near kitchen. **Warning**
01B-03-5    High Priority - Stop Sale issued due to adulteration of food product. Cook handled raw beef after handling soiled dishes and dish water at triple sink.
41-27-4    High Priority - Wiping cloth sanitizer solution exceeds the maximum concentration allowed. Red sanitizer bucket by server station chlorine sanitizer 200+ppm. Solution discarded. **Corrected On-Site**
12A-03-4    Intermediate - Employee washed hands in a sink other than an approved handwash sink. Two cooks washed hands in triple sink. Manager coached employee on proper hand washing procedures. **Warning**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Interior of ice machine in kitchen. Interior of reach in cooler in server area. **Repeat Violation**
31A-04-4    Intermediate - Handwash sink removed from food preparation/dishwashing area. Hand washing sink by ice machine in kitchen is present, however drain pipe for sink is leaking sewage onto floor surface. Sink is marked out of order and plumber is dispatched to repair. **Corrective Action Taken** **Warning**
