A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Dead roaches on premises.
Observed 13 dead roaches in establishment near service bar and server stations in dining room. Dead roaches discarded. **Corrected On-Site** **Warning**
Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items.
Employee shirts stored with sauces in to go area. Shirts moved and stored correctly. **Corrected On-Site**
Basic - Equipment and utensils not properly air-dried - wet nesting.
Clean plastic stored on rack/shelf near bar area. Cups removed to wash. **Corrected On-Site** **Repeat Violation**
Basic - Equipment in poor repair.
Two microwaves on cook line missing handles. Manager states parts are on order. **Corrective Action Taken** **Repeat Violation**
Basic - Floors, walls, and/or ceilings soiled/has accumulation of debris.
Floor under server station equipment.
Floor under fryers in kitchen.
Walls in dish area.
Ceiling vents in kitchen area. **Repeat Violation**
Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust.
Floor fan in kitchen.
Area around soda nozzles in server station near bar.
Hood filters above cooking equipment.
Racks in walk in freezer. **Repeat Violation**
High Priority - Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands.
Server brought soiled dishes and placed in dish area, and immediately handled food/clean plates from pass through window without washing hands. Manager coached employee on proper hand washing procedures. **Corrective Action Taken** **Repeat Violation** **Admin Complaint**
High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area.
Observed approximately 41 live flying insects in establishment: approximately four in main kitchen area, 18 in service bar/bar areas, 17 in back liquor storage room, and two on beer walk in cooler. **Repeat Violation** **Admin Complaint**
High Priority - Pesticide-emitting strip present in food prep area.
Strip in server station where drinks are prepared. Strip discarded. **Corrected On-Site**
High Priority - Roach activity present as evidenced by live roaches found.
Observed one live roach in server station near front door in dining area. Roaches killed and discarded. **Corrected On-Site** **Warning**
High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit.
On main cook line in flip top coolers, guacamole, cheese, eggs, cut lettuce, cut cabbage 52F. All items were at correct temperature approximately three hours prior. Flip top coolers were opened frequently during lunch. All items moved to walk in cooler to cool. **Warning**
High Priority - Toxic substance/chemical improperly stored.
Spray bottle containing yellow chemical cleaner stored on shelf with sauces in to go area. Bottle moved and stored correctly. **Corrected On-Site** **Repeat Violation** **Admin Complaint**
