Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

05-09-4    Basic - No conspicuously located ambient air temperature thermometer in holding unit- refrigerator unit next to freezer located at end of steam table.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gaskets to two door reach in freezer unclean.
22-16-4    Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Interior to two door reach in freezer unclean.
50-17-2    High Priority - Operating with an expired Division of Hotels and Restaurants license.
35A-09-4    High Priority - Presence of insects, rodents, or other pests. Thirteen live ants on two door reach in freezer used as prep table.
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Two live roaches in oven. One live roach on wall above stove. One live roach at prep table in front of steam table.
03B-01-6    High Priority - Time/temperature control for safety food, other than whole meat roast, hot held at less than 135 degrees Fahrenheit. chicken (110F - Hot Holding) cooked at 7 am ; rice (78F - Hot Holding) in steam table. Operator states food cooked 2.5 hours prior and placed in steam table. Operator reheated food items to 165F on stove. **Corrective Action Taken**
31A-09-4    Intermediate - Handwash sink not accessible for employee use at all times. Prep table stored directly in front of hand sink in kitchen.

16-37-1    Intermediate - No chemical test kit provided when using sanitizer at three-compartment sink/warewashing machine or wiping cloths-quaternary.
05-08-4    Intermediate - No probe thermometer provided to measure temperature of food products.
