Violations:
A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Accumulation of black/green mold-like substance in the interior of the ice machine/bin.
Observed the ice machine and shroud have foreign substance on them.
Basic - Dead roaches on premises.
Observed a dead roach stuck to the side of a tea urn, front line.
ALSO, a dead roach in the front line cabinet under the right end coffee/tea area.
Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items.
Observed a purse stored on top of the food warmer in the kitchen.
Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust.
Observed around the front line hand wash sink an accumulation of wet soil/debris.
ALSO, the area under the grill has grease accumulation.
ALSO, multiple gaskets throughout the restaurant have moist mold like substance on them.
ALSO, drawers and cabinets have accumulation of debris in them.
ALSO, dried food debris was under the front line counter top equipment.
ALSO, the coffee caddys have an accumulation of dried food debris. This item:

Basic - Wet wiping cloths used for occasional spills on equipment food- and nonfood-contact surfaces not clean.
Observed the front line sanitizer with soiled water.
Basic - Worn, torn and/or soiled floors/carpeting.
Observed the floor under equipment in the front and back have an accumulation of buildup and debris.
High Priority - Chlorine sanitizer in spray bottle/bucket not at proper minimum strength.
Observed 2 front line sanitizer buckets at 0 ppm. **Repeat Violation** **Admin Complaint**
High Priority - Dishmachine not sanitizing properly. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired and sanitizing properly.
Observed the hi-temp dish wash machine only reached 123F. **Repeat Violation** **Admin Complaint**
High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area.
Observed a fly at the office door in the kitchen.
ALSO, 1 fly on the front line by the dish wash machine.
High Priority - Roach activity present as evidenced by live roaches found.
Observed a live roach on the front line reach-in cooler at the opening of the front line. **Admin Complaint**
High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit.
Observed on the front line: diced pork (60F - Cold Holding); cut tomato (64F - Cold Holding); sliced ham (60F - Cold Holding), unit was then turned on. **Corrective Action Taken** **Repeat Violation** **Admin Complaint**
High Priority - Time/temperature control for safety food identified in the written procedure as a food held using time as a public health control has no time marking and the time removed from temperature control cannot be determined.
Observed no time mark for eggs out on the cook line. An adjusted time mark was added. **Corrected On-Site**
Intermediate - Handwash sink used for purposes other than handwashing.
Observed the front line used to rinse a cloth.
ALSO, the kitchen hand wash sink had Raid Max in it. This was removed. **Corrected On-Site**
