Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

36-73-4    Basic - Floors, walls, and/or ceilings soiled/has accumulation of debris. Floor under drink dispenser near front counter. Floor under drive through drink dispenser. Floor under cooking/refrigeration equipment in kitchen. Floor under racks in dry storage area. Wall beside drink dispenser in dining area. Wall in dry storage area by bag in box soda rack.
38-07-4    Basic - Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. Light fixture in dry storage area.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Hood filters above cooking equipment. Racks in walk in cooler.
16-46-4    Basic - Old labels stuck to food containers after cleaning. Clean storage pans stored on rack/shelf near dish area.
33-16-4    Basic - Open dumpster lid. **Repeat Violation**
21-08-4    Basic - Wiping cloth quaternary ammonium compound sanitizing solution not at proper minimum strength. Sanitizer bucket in kitchen tested 0ppm. Manager discarded solution and remade. Solution tested 200ppm. **Corrected On-Site**
02D-01-5    Basic - Working containers of food removed from original container not identified by common name. Large container of breading by fryers. Manager labeled. **Corrected On-Site**
12A-13-4    High Priority - Employee handled soiled equipment or utensils and then engaged in food preparation, handled clean equipment or utensils, or touched unwrapped single-service items without washing hands. Employee working in front counter area handled personal cell phone and immediately handled single use items without washing hands. Manager coached employee on proper hand washing procedures. **Corrective Action Taken**
12A-17-4    High Priority - Employee rubbed hands together for less than 10-15 seconds while washing hands. Employee working in dining area came behind counter to wash hands, but only washed hands for approximately four seconds. Employee re-washed hands properly. **Corrective Action Taken**
35A-05-4    High Priority - Roach activity present as evidenced by live roaches found. Observed five live roaches in establishment: three behind drink dispenser inside wall (wall is cut open) connecting to kitchen, and two behind garbage receptacle across from drink dispenser in dining area. Manager killed one live roach behind garbage receptacle. **Corrective Action Taken** **Warning**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. Coleslaw in main make line 57F. Slaw placed into service approximately two hours prior. Slaw moved to walk in cooler to cool. **Corrective Action Taken**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Ice chute for drink dispenser in dining area. **Repeat Violation**
51-16-7    Intermediate - No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days. Back storage area behind mop sink and little storage area by drive through have been added. **Warning**
53B-13-5    Intermediate - Proof of required state approved employee training not available for some employees. To order approved program food safety material, call DBPR contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. Missing for one employee working today.
