Violations:
A summary of the violations found during the inspection are listed below.
The department cites violations of Florida's sanitation and safety laws,
which are based on the standards of U.S. Food and Drug Administration's Food Code.
High Priority violations are those which could contribute directly to a foodborne
illness or injury and include items such as cooking, reheating, cooling and hand-washing.
Intermediate violations are those which, if not addressed, could lead to risk factors
that contribute to foodborne illness or injury. These violations include personnel training,
documentation or record keeping and labeling. Basic violations are those which are considered
best practices to implement. While most establishments correct all violations in a timely
manner (often during the inspection), the division's procedures are designed to compel
compliance with all violations through follow-up visits, administration action or closure
when necessary.
Basic - Accumulation of dead or trapped birds, insects, rodents, or other pests, in control devices.
Observed approximately 250 dead flies and small roaches on sticky traps hanging throughout dry storage area and fish prep area. **Admin Complaint**
Basic - Dead roaches on premises.
Observed dead roaches in the following areas:
Approximately 80 dead roaches in the dry storage area on floor and on shelving.
Approximately 25 on kitchen floor. **Admin Complaint**
Basic - Reach-in cooler interior/shelves have accumulation of soil residues.
Observed reach in cooler behind with soil build up at bottom and in door tract.
High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area.
Observed flying insects in the following areas:
8 in the fish prep area
7 in the bar area
2 in the dish wash area

High Priority - Raw animal food stored over/not properly separated from ready-to-eat food.
Observed raw shell oysters stored on top of ready to serve beer in cans. Employee moved oysters and stored properly. **Corrected On-Site**
High Priority - Roach activity present as evidenced by live roaches found.
Observed 10 live roaches on floor in dry storage area and 6 live roaches on floor in kitchen. **Admin Complaint**
