Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

12B-07-4    Basic - Employee beverage container on a food preparation table or over/next to clean equipment/utensils. 1) drinks on prep table next to establishments food. Discussed with employee. **Repeat Violation**
40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. 1) employee personal items on prep table near food. Discussed with employee. **Repeat Violation**
36-01-4    Basic - Floor, wall, ceiling not cleaned when the least amount of food is exposed. 1) floor throughout kitchen and under equipment extremely soiled with hair, food debris, cardboard. 2) walls behind triple sink soiled with black/green mold like substance. 3) walls behind toaster oven near main flip-top cooler soiled with grease. 4) ceiling vents in kitchen soiled with dust. 5) Floor drains in kitchen and front counter soiled with black/green mold like substance, food debris, and plastic gloves causing flying insects to breed.

23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. 1) Gaskets on walk-in cooler, flip-top coolers soiled with food debris and black/green mold like substance. **Repeat Violation**
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. 1) 20 live flying insects at floor drain by front counter fruit flip-top cooler. 2) 20 live flying insects at floor drain by triple sink. 3) 10 flying insects at nonfunctional hand washing station in prep area. 3) 10 flying insects breading under cardboard shoved under prep table 4) 5 flying insects flying around dry storage landing on boxes. **Warning**
