Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

14-01-5    Basic - Bowl or other container with no handle used to dispense food. Cup without handle used to dispense cheese on cookline. Also, container without handle used to dispense sauce in walk-in-cooler. Employee removed both cup and container from foods. **Corrected On-Site**
14-74-7    Basic - Cold holding equipment not maintained in good repair. Do not store time/temperature control for safety food in this unit until the unit is repaired. Reach-in cooler across from stove unable to maintain temperature of 41F and below. Ambient temperature 46F. Operator stated he had doors and lid opened and closed alot this morning. Observed dust build up on fan and lid sitting on top of cutting board causing lid not to seal in cold air. Employee moved cutting board for lid to fully close and operator stated he will have fan area cleaned today **Corrective Action Taken**
08B-38-4    Basic - Food stored on floor. Container of oil in back prep area stored on floor.
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Gaskets on reach-in coolers on cook line and gasket on walk-in-cooler soiled.
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry. Wet mop stored in mop bucket unable to properly dry.
50-17-3    High Priority - Operating with an expired Division of Hotels and Restaurants license. Establishment license delinquent, active. Operator stated he paid in August. Operator still owes fee and working on trying to pay it during this inspection. **Corrective Action Taken**
03A-02-5    High Priority - Time/temperature control for safety food cold held at greater than 41 degrees Fahrenheit. feta cheese crumbles (46F - Cold Holding) in top of reach-in cooler across from stove. Employee stated cheese was placed in cooler at 11a. Employee stated he had lid opened and closed a lot this morning. Employee stated he will keep lid closed. Also, sliced mozzarella cheese (45F - Cold Holding) in bottom of reach-in cooler across from stove. Employee stated he will keep door closed to chill cheese. **Corrective Action Taken**
02C-03-5    Intermediate - Commercially processed ready-to-eat, time/temperature control for safety food opened and held more than 24 hours not properly date marked after opening. Commercially packaged taziki sauce and heavy whipping cream in walk in cooler opened more than 24 hours not date marked.
11-27-4    Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Operator unable to provide written procedures for clean up events. Distributed Training material during this inspection.
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Cutting board on reach in cooler across from stove soiled/ stained. Also, soda nozzles at self dispensing soda machine in dining area soiled. Also, ice chute at self dispensing soda machine soiled.
11-26-1    Intermediate - No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Operator unable to provide proof that employees are informed of reporting symptoms related to food borne illnesses to person in charge. Distributed health form to operator during this inspection
