Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

16-23-4    Basic - Accumulation of lime scale on the inside of the dishmachine.
23-24-4    Basic - Buildup of food debris/soil residue on equipment door handles of reach in coolers at front counter.
36-34-5    Basic - Ceiling/ceiling tiles/vents soiled with accumulated food debris, grease, dust, or mold-like substance. Observed ceiling vents over standing reach in cooler located in dry storage area with accumulated dust and debris.
13-03-4    Basic - Employee with no hair restraint while engaging in food preparation.
14-11-5    Basic - Equipment in poor repair. Observed torn gaskets on reach in cooler located at front counter.
36-22-4    Basic - Floor area(s) covered with standing water. Observed standing water under counter on floor at front counter.
36-73-4    Basic - Floor soiled/has accumulation of debris. Observed floors under dishwashing machine and under equipment with build up of mold like substance and debris.
08B-38-4    Basic - Food stored on floor. Observed boxes of salt caramel sauce on floor in dry storage area.
02D-01-5    Basic - Observed Working containers of cake batter removed from original container not identified by common name.
22-16-4    Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Observed reach in cooler shelves and gaskets with mold like substance.
36-27-5    Basic - Wall soiled with accumulated grease, food debris, and/or dust. Observed walls under dish machine with accumulated mold like substance.
42-01-4    Basic - Wet mop not stored in a manner to allow the mop to dry.
22-57-6    High Priority - Heat strip failed to turn black to indicate the sanitization temperature of 160 degrees Fahrenheit was achieved on the dish surface and/or high temperature holding thermometer did not reach 160F. Discontinue use of dishmachine for sanitizing and set up manual sanitization until dishmachine is repaired.
35A-02-6    High Priority - Live, small flying insects in kitchen, food preparation area, food storage area and/or bar area. Observed approximately 25- 30 or more flying around the dry storage area on bananas, walls, boxes with to go sealed cups, on dish rack with clean sanitized pans and utensils and on unwrapped paper towels shelves and under the dish machine. Observed 3 or more flies flying around floor at front counter.
