Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

40-06-5    Basic - Employee personal items stored in or above a food preparation area, food, clean equipment and utensils, or single-service items. Jackets and purse on rack with to go containers.
13-07-4    Basic - Employee wearing jewelry other than a plain ring on their hands/arms while preparing food. Wearing watch.
24-08-4    Basic - Equipment and utensils not properly air-dried - wet nesting. Metal pans on clean storage.
14-11-5    Basic - Equipment in poor repair. Ice machine gasket and paper towel dispenser broken. **Repeat Violation**
08B-47-4    Basic - Food not stored at least 6 inches off of the floor. Gyro cones in walk in cooler. **Repeat Violation**
14-69-4    Basic - Ice buildup in reach-in freezer and/or walk-in freezer. Walk in freezer on floor and behind fans.
10-08-5    Basic - Ice scoop handle in contact with ice. Ice bin at drive through.
38-07-4    Basic - Lights in food preparation, food storage or warewashing area missing the proper shield, sleeve coatings or covers. Throughout kitchen area. **Repeat Violation**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Outside of grease trap heavy grease buildup.
33-16-4    Basic - Open dumpster lid.
32-23-4    Basic - Toilet/urinal not flushing/functioning properly. Urinal covered, out of order. **Repeat Violation**
14-17-4    Basic - Walk-in cooler/freezer shelves with rust that has pitted the surface. Walk in freezer. **Repeat Violation**
03C-97-2    High Priority - Gyro meat in a broiler that is turned off. Operator turned broiler back on. **Corrected On-Site**
11-27-4    Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. Provided operator with form. **Corrected On-Site**
22-02-4    Intermediate - Food-contact surface soiled with food debris, mold-like substance or slime. Outside of bulk containers soiled old food debris. **Repeat Violation**
11-26-1    Intermediate - No proof provided that food employees are informed of their responsibility to report to the person in charge information about their health and activities related to foodborne illnesses. Provided operator with form. Form must be filled out by each individual employee and kept on file in facility. **Corrective Action Taken**
