Violations:
A summary of the violations found during the inspection are listed below. The department cites violations of Florida's sanitation and safety laws, which are based on the standards of U.S. Food and Drug Administration's Food Code. High Priority violations are those which could contribute directly to a foodborne illness or injury and include items such as cooking, reheating, cooling and hand-washing. Intermediate violations are those which, if not addressed, could lead to risk factors that contribute to foodborne illness or injury. These violations include personnel training, documentation or record keeping and labeling. Basic violations are those which are considered best practices to implement. While most establishments correct all violations in a timely manner (often during the inspection), the division's procedures are designed to compel compliance with all violations through follow-up visits, administration action or closure when necessary.

35A-03-4    Basic - Dead roaches on premises. Four dead roaches in restroom in kitchen. Dead roaches discarded. **Corrected On-Site**
36-73-4    Basic - Floors, walls, and/or ceilings soiled/has accumulation of debris. Floor under equipment at front counter. Floor in small area by walk in freezer. Floor under equipment in kitchen. Wall under triple sink in kitchen. **Repeat Violation**
08B-38-4    Basic - Food stored on floor. Case of shrimp on floor in walk in freezer. **Repeat Violation**
05-09-4    Basic - No conspicuously located ambient air temperature thermometer in holding unit. Missing in multiple reach in coolers. **Repeat Violation**
14-47-4    Basic - Nonfood-contact equipment exposed to splash/spillage not constructed of materials that are corrosion resistant, non-absorbent, smooth and easily cleanable. Interior of walk in freezer walls/door are rusted. Shelf below prep table in kitchen is rusted. Two burners by fryers are rusted. **Repeat Violation**
23-03-4    Basic - Nonfood-contact surface soiled with grease, food debris, dirt, slime or dust. Cabinet area under register at front counter. Gaskets for walk in cooler, walk in freezer and reach in coolers in kitchen. Inside light fixture by walk in freezer. Sides of fryers. Hood filters above cooking equipment. **Repeat Violation**
22-16-4    Basic - Reach-in cooler interior/shelves have accumulation of soil residues. Interior of reach in coolers in kitchen.

01C-03-4    Intermediate - Clam/mussel/oyster tags not marked with last date served. Some tags are missing dates. **Repeat Violation**
11-27-4    Intermediate - Establishment has no written procedures for employees to follow in response to a vomiting or diarrheal event where the vomit or diarrhea is discharged onto surfaces in the establishment. No plan could be provided. Inspector emailed example of plan. **Corrective Action Taken**
31B-02-4    Intermediate - No paper towels or mechanical hand drying device provided at handwash sink. At hand washing sink in restroom.
51-16-7    Intermediate - No plan review submitted and approved - renovations were made or are in progress. Must submit plans and plan review application to DBPR H and R Plan Review office located at 2601 Blair Stone Rd., Tallahassee, FL 32399-1011. Plans must be submitted AND approved within 60 days.
